Preventive Exam/Screening Frequently Asked Questions
A preventive visit is a yearly appointment with your primary care provider intended to prevent illnesses and detect health concerns early before symptoms are noticeable. The services provided are usually based on age, but you can expect a discussion around your current health, family history, past illnesses, surgeries, and chronic conditions. A physical exam will likely include checking your vital signs, listening to your heart and lungs, checking your ears, eyes, throat, and abdomen.
Biometric screenings are short health exams that include bloodwork to measure cholesterol, triglycerides and glucose levels and measurements like height, weight, and waist circumference. This will give you baseline measurements, however we recommend following up with a primary care provider to review these numbers and address any concerns. You will also be screened for nicotine use to fulfill the tobacco free step of the wellness rate.
Please ask your provider when scheduling if there are any specific instructions around eating or drinking prior to your appointment. It is likely that your provider will ask that you fast (aside from water) for 8 – 12 hours prior to your appointment time to capture the most accurate bloodwork. Continue to take medications as prescribed unless notified to do this differently.

This information is between you, your provider, and Virgin Pulse. Trilogy Health Services does not maintain your personal health information. Your information will never be used to determine health care coverage or employment status.
Virgin Pulse will only honor a form printed from the individual’s account with the Virgin Pulse identifiers auto populated at the top of the form. You cannot use a generic form and submit with blanks and or handwritten information in the fields.
To receive your points, you must send in the completed Virgin Pulse Biometric Screening Verified Form within 6 months of completing the lab draw/physician’s visit. You can submit the form in two ways to Virgin Pulse.
- Upload directly to your Virgin Pulse Account: Your completed Virgin Pulse Biometric Screening Verified Form can be uploaded by signing into your account, click on Support and select Submit a request. Then choose the appropriate form option from the drop-down menu. Results take up to 2 weeks to be credited in your VP account.
- Fax: 508-302-0055. Results take up to 2 weeks to be credited to your VP account.
Once the form is loaded into the system, you will see this requirement marked Complete on your VP Rewards page.
You will not need to take any action as LabCorp will send your results directly to Virgin Pulse on your behalf.
The in-range results generally follow the recommendations from the U.S. Preventive Services Taskforce.

MetLife provides a $50 Health Screening Benefit (HSB) to enrollees who provide proof of an annual wellness screening. The HSB can be redeemed for employees, and their dependents, covered under MetLife’s Critical Illness or Accident Coverage Plans. The $50 is provided for each type of coverage, so if you are enrolled in both plans, you will receive two $50 checks, just for submitting proof of screening. Simply call and provide verbal verification of completion over the phone or fill out the attached HSB Form.
